SUMMARY:2025 Practice Track Meetings for EPT Practices
DESCRIPTION:Practice tracks are groups of EPT practices that meet virtually for 90 minutes either monthly or every other month. Agendas are set collaboratively by attendees and facilitators and include interactive content presentations and spaces for peer sharing and collaboration. Each meeting is facilitated by a dedicated practice track facilitator. Facilitators include the California Medical Association\, California Primary Care Association\, Indigenous Pact\, Institute for High Quality Care\, and Thatcher Consulting. \nDownload this spreadsheet to find the list of practice tracks and corresponding meeting dates through July 2025.  Additionally\, we encourage EPT team members to keep in touch with their practice track on an ongoing basis by logging in to PopHealth+ and opening “Groups” on the left-side menu. \nEPT practices should already have received a calendar appointment for their practice track\, sent from their practice track facilitator. We will record the sessions and make the materials available for both tracks\, so if you’re unable to attend one\, you can still access the content later. \nPlease register by Friday\, January 24.  Please see our FAQs at the bottom of this page if you have questions about the Learning Session. \nAccess Track\nLed by Coleman Associates\, participants will dive into advanced schedule management strategies to enhance capacity utilization and access to care. This interactive session equips learners with the tools to critically evaluate their current scheduling practices and identify the most effective tactics tailored to their unique site demands and service lines. \nThrough practical exercises and collaborative discussions\, this session provides a clear pathway for learners to create actionable changes that improve access to care while maintaining operational efficiency. \nBy joining this session\, practices will: \n\nUnderstand which schedule management tactics work best to improve capacity utilization and access to care.\nBe able to evaluate and determine which strategies are most applicable to their site and service lines based on demand & supply to increase capacity utilization.\nBe able to design and trial a new schedule template to improve patient access and maintain goal continuity to care.\nDevelop communication tactics to engage colleagues in schedule template changes to ensure adoption.\n\nThe Access track is approved for 3.75 CME\, CNE\, CPE\, ADA or ACE credits or 0.4 CEUs. Download the continuing education information. \nRegister for the Access Track\nData Track\nLed by the Learning Center\, UCSF Center for Excellence in Primary Care (CEPC)\, and Blue Path Health\, this interactive session will explore data management and equity-focused care to support practices in meeting the May 2025 deliverables\, including: \n\nWhat’s needed to meet key upcoming EPT milestones\, including the data implementation plan and how to stratify and calculate HEDIS-like measures.\nHow to stratify data to identify disparities.\nBest practices for collecting accurate data on Race\, Ethnicity\, Language (REAL)\, and Sexual Orientation and Gender Identity (SOGI).\nHow to leverage internal and external data sources to improve measure performance.\nHow data management can be improved through the California Data Exchange Framework (DxF).\nBasics of Data Sharing Agreements and what they involve.\nHow to identify potential partners for data sharing\, including the state’s Qualified Health Information Organizations (QHIOs).\n\nCEUs can be provided for nursing staff and behavioral health staff – we will provide additional information on the number of contact hours closer to the session. The Learning Center and our partners are exploring how to provide CE for MDs at future learning communities. \nRegister for the Data Track\n  \nFREQUENTLY ASKED QUESTIONS  \nEvent Information \nCan my team split up and attend different tracks?\nWe encourage practices to have multiple members of their EPT team join the same learning session track. If your practice would like to work on Access and Data at the same time\, the Learning Center recommends that multiple team members (2 or more) attend each track at the Learning Session. \nCan I attend part of one track and part of the other track?\nNo. The sessions have been carefully designed to layer content pieces. It will be confusing to join mid-way through. \nAre continuing education credits available for Learning Sessions?\nCurrently\, the Access track is approved for 3.75 CME\, CNE\, CPE\, ADA or ACE credits or 0.4 CEUs. For the Data Track\, CEUs can be provided for nursing staff and behavioral health staff. We will provide additional information on the number of contact hours closer to the Learning Community. We are exploring how to provide CE for MDs at future learning communities. \nI’m staff at one of the Managed Care Plan (MCP) sponsors. How many attendees from my MCP can attend?\nThree staff from each MCP may attend either track at their preferred Learning Session. We recommend that MCPs attend the session that all or most of their sponsored practices are attending. \nHow do I know which Learning Community my practice is in?\nTo find out which Learning Community a practice is assigned to\, visit PopHealth+ and open “Groups” on the left-side menu or download our EPT Practice Assignments spreadsheet. \nTechnical Questions  \nWill sessions be recorded so that we can listen to them asynchronously (e.g.\, not live)? \nYes\, sessions will be recorded. All materials will be made available after. \nWhen will I get a calendar invite and/or webinar connection information? Clicking on one of the registration links above will redirect you to the Zoom Meeting Registration for that track. Please fill out the registration form and Zoom will send you a confirmation email. You can download the calendar invite from the Zoom confirmation email. \nEPT Focus Areas & Technical Assistance  \nHow will the Learning Session prepare my practice for the deliverables submission? \nPractices will receive information and materials needed to complete the deliverables prior to the Learning Session. Learning Sessions will reinforce this content\, provide an opportunity to learn live with experts\, teach advanced concepts\, and answer your questions. \nParticipation  \nOur team already knows these content areas\, or we have a conflict with the time. Do we still need to attend the Learning Session? \nYes! The Learning Center developed participation guidelines based on three considerations: (1) engagement in the training is beneficial for success; (2) creating a culture of “all share all learn” benefits the entire EPT community; and (3) even practices with prior knowledge may still benefit from guidance on how to adjust their current capabilities to effectively meet the milestones and deliverables for EPT. If you have a conflict with your Learning Session\, you may attend one of the other two Learning Sessions or access the materials when they are posted online. \nWhat are the EPT participation requirements? \nThe Learning Center recommends that practices have representation at 80% of EPT events—Learning Sessions and Practice Tracks—from at least two people. Small practices with 1-3 FTEs may have one attendee. Practices must submit deliverables related to milestones released in year 1 (2024) by the end of year 2 (2025).

This information page is specific to the Sequoia Learning Community. \nThe Learning Session on February 24th offers two tracks: Data and Access. To maximize the benefits for your practice\, we suggest that at least two members of your EPT team join the same track.
