Identify two benefits of enhanced team-based models of care\n2. Describe approaches to prepare staff for new team-based roles\n3. Name strategies to create a culture shift supportive of team culture \nWho should join: Multiple team members\, you will have the opportunity to review your health practices care team in real time. \nRegister Here. \n  \nThis course meets the qualifications for 1.0 contact hour of continuing education credit for nurses as required by the California Board of Registered Nursing\, Provider #18090. You must attend the full session in order to qualify for contact hours. Join the University of California Center for Excellence in Primary Care for an in-depth\, skills-based training on the HEAR Technique—a brief\, respectful approach to engaging patients who are hesitant about vaccines\, cancer screenings\, and other preventive care. Participants will practice the technique and learn how to address common patient questions with confidence. \nJoin us on Wednesday\, January 21st from 12:30pm – 2pm. \nRegister Here. \n  \nLearning Objectives: \nAt the end of this session\, participations will be able to… ​ \n\nUtilize communication techniques to promote vaccine acceptance\nDescribe the four steps of the HEAR Technique\nApply the HEAR Technique to address ambivalence and diffuse challenging conversations\n\n  \nPresenters from the UCSF Center for Excellence in Primary Care include:\nRachel Willard-Grace\, MPH\, Director\nPatricia Mejía\, Associate Director of Training\nMeghan Elliott\, Trainer and Communications Coordinator \n  \nThis course meets the qualifications for 1.0 contact hour of continuing education credit for nurses as required by the California Board of Registered Nursing\, Provider #18090. You must attend the full session in order to qualify for contact hours. Course completion certificates will be awarded upon completion of course evaluations. Documentation must be retained by the Participant for a period of four years after the conclusion of this program. \nThe UCSF Center for Excellence in Primary Care is approved by the California Association of Marriage and Family Therapists (CAMFT) to sponsor continuing education for LMFTs\, LCSWs\, LPCCs\, and/or LEPs. The UCSF Center for Excellence in Primary Care maintains responsibility for this program/course and its content. This course meets the qualifications for 1.0 hour of continuing education credit for LMFTs\, LCSWs\, LPCCs\, and/or LEPs as required by the California Board of Behavioral Sciences\, Provider #1032866. SUMMARY:Improving Performance through Data on the CIS Combo 10 Childhood Immunization Measure
DESCRIPTION:Improving performance on the CIS Combo 10 childhood immunization quality measure requires more than clinical effort—it depends on effective data exchange\, accurate patient matching\, and coordinated workflows and data sharing across providers\, hospitals\, payers\, and public health agencies. This webinar shares key learnings and implementation insights from a multi-session CIS Combo 10 Data Exchange Design Studio led by Connecting for Better Health (C4BH) and the Population Health Learning Center (PHLC) and funded by Covered California. The design studio brought together practices\, payers\, the California Immunization Registry\, and a county public health department to examine real-world barriers to immunization data quality and completeness. The session is designed for providers\, health plans\, public health agencies\, health IT partners\, and quality improvement teams interested in improving performance on the CIS Combo 10 childhood immunization measure. \nParticipants will learn: \n\nCommon challenges impacting CIS Combo 10 performance\, including patient identity resolution\, data quality and completeness\, and patient authorization for immunization data sharing\nPractical recommendations and implementation guidance drawn from current-state and future-state workflow designs\, use cases\, and system flow diagrams\nHow improved data exchange can support practices\, plans\, and public health agencies in closing immunization gaps and achieving quality measure goals\n\nRegister Here.
