SUMMARY:2025 Practice Track Meetings for EPT Practices
DESCRIPTION:Practice tracks are groups of EPT practices that meet virtually for 90 minutes either monthly or every other month. Agendas are set collaboratively by attendees and facilitators and include interactive content presentations and spaces for peer sharing and collaboration. Each meeting is facilitated by a dedicated practice track facilitator. Facilitators include the California Medical Association\, California Primary Care Association\, Indigenous Pact\, Institute for High Quality Care\, and Thatcher Consulting. \nDownload this spreadsheet to find the list of practice tracks and corresponding meeting dates through July 2025.  Additionally\, we encourage EPT team members to keep in touch with their practice track on an ongoing basis by logging in to PopHealth+ and opening “Groups” on the left-side menu. \nEPT practices should already have received a calendar appointment for their practice track\, sent from their practice track facilitator. If you are missing the calendar appointment\, please email your practice track facilitator. General questions about practice tracks can be directed to the Learning Center at info@pophealthlc.org.
