SUMMARY:It Takes a Village: Partnering to Improve Colorectal Cancer Screenings and COL Performance
DESCRIPTION:Early detection of colorectal cancer is key to prevention and better outcomes. Many practices face low colorectal cancer screening compliance\, while 1 in 5 diagnoses now occurs in individuals under age 55\, making it the leading cause of cancer-related death among young adults. This webinar shares key learnings and implementation insights from a six session Colorectal Cancer Screening (COL) Data Exchange Design Studio led by Connecting for Better Health (C4BH) and the Population Health Learning Center (PHLC) and funded by Covered California. This Design Studio brought together six provider practices\, four managed care plans\, and Exact Sciences—a diagnostics organization and owner of the Cologuard product—to identify opportunities to increase compliance\, strengthen patient and provider education\, and enhance data-sharing capabilities.
