This resource uses fictional patient stories to illustrate how depression screening and follow-up information moves across practices, behavioral health providers, health plans, and other care partners. The scenarios follow patients from an initial depression screen through additional assessment, referral, treatment, follow-up, and closure of the Depression Screening and Follow-Up quality measure gap. They highlight opportunities to improve documentation, coding, data exchange, workflow coordination, and the timely capture of qualifying follow-up services.
