We partner with California DHCS to provide full-service program design and implementation to strengthen population health management, advance health equity, and improve quality and value for Medi-Cal enrollees.
We work with 200+ primary care practices collectively providing care to 2 million+ Medi-Cal enrollees through practical tool development, learning, coaching, and statewide peer exchange.
