This completed example shows how Prime Wellness approached the EPT Disparity Reduction Plan milestone. It demonstrates how a practice can review stratified performance data, identify a priority health disparity, define the patient population most affected, and examine contributing factors. Practices can use the example to understand the expected level of detail for establishing an improvement goal, selecting interventions, defining implementation activities, assigning responsibilities, identifying resources and partnerships, and measuring progress.
