The scenario highlights potential breakdowns in identifying a delivery, notifying the appropriate care team, scheduling timely follow-up, exchanging clinical information, and documenting completed postpartum care. Practices and partners can use this resource to examine current workflows, clarify organizational responsibilities, and identify data-sharing improvements that support care coordination and postpartum quality-measure performance. The user story was developed through a postpartum care community design process involving EPT practices, managed care plans, the California Department of Health Care Services, Connecting for Better Health, and the PopHealth Learning Center.
