This process map illustrates how postpartum care information and activities move across hospitals, obstetric and primary care providers, managed care plans, patients, and other care partners. It highlights key steps from delivery notification and patient identification through outreach, appointment scheduling, follow-up care, documentation, and quality-measure closure. Practices and partners can use the map to examine current workflows, identify where information may be delayed or incomplete, clarify organizational responsibilities, and design improvements that support timely postpartum care and more reliable data exchange. The resource was developed through a co-design process involving EPT practices, managed care plans, the California Department of Health Care Services, Connecting for Better Health, and the PopHealth Learning Center.
