This user story follows Yolanda Ortega, a fictional postpartum patient, to illustrate how postpartum care information moves across hospitals, obstetric and primary care providers, managed care plans, and other care partners. The scenario highlights opportunities to improve delivery notification, patient identification, outreach, appointment scheduling, clinical information exchange, follow-up documentation, and closure of postpartum care quality gaps. Practices and partners can use this resource to examine current workflows, clarify responsibilities across organizations, and identify data-sharing improvements that support timely, coordinated postpartum care. The resource was developed through a co-design process involving EPT practices, managed care plans, the California Department of Health Care Services, Connecting for Better Health, and the PopHealth Learning Center.
