These slides are from Session 4 of the HealthBegins Health-Related Social Needs Skill-Building Workshop Series. The session focuses on strengthening referral pathways and building reliable connections between practices, community-based organizations, health plans, and other service partners. It provides guidance for defining staff responsibilities, coordinating referrals, supporting patients through the referral process, exchanging information with partners, and tracking whether referrals reach a documented outcome.
