This session provides practical, actionable guidance on navigating referral pathways and building CBO partnerships under CalAIM, designed specifically for Federally Qualified Health Centers and practices with existing organizational infrastructure and MCP relationships. Content assumes foundational CalAIM familiarity and focuses on operationalizing referral workflows and formalizing Community partnerships at scale.

Learning Objectives
Participants will be able to:
– Navigate the full referral pathway into CalAIM ECM and Community Supports through their MCP
– Distinguish between MCP-mediated referral pathways and direct Community-Based referral options, and identify when each is appropriate
– Describe closed-loop referral — what it means, how it works within MCP systems, and what FQHCs should expect in terms of follow-up
– Identify strategies for formalizing Community-Based partnerships, including MOUs, co-location agreements, and data-sharing protocols
– Understand the co-location model as a practical partnership example
– Learn from other similar organizations on how they have incorporated these services into their practice. “
