This session covers the same core topic as Part 2A, referral pathways and Community-Based partnerships, but is specifically tailored to solo and independent practices, with a focus on lower-lift entry points and practical tools for practices with limited administrative capacity, smaller patient panels, and fewer existing MCP relationships.
Learning Objectives
Participants will be able to:
– Identify at least two practical, lower-lift entry points into CalAIM referral pathways that do not require dedicated care coordination infrastructure
– Understand what closed-loop referral means in the context of a small practice and what to realistically expect from MCP follow-through
– Identify local CBOs and CalAIM-enrolled community partners that can serve as referral destinations without requiring a formal partnership agreement
– Describe informal CBO partnership models that are feasible for SIPs — including warm handoffs, shared resource guides, and community liaison relationships “
