Helping primary care practices build stronger systems for population health
Health Plan of San Mateo (HPSM) has committed $60 million over five years to invest in its primary care network and the providers, workforce, and members it serves. As part of that strategy, the Population Health Learning Center (The Learning Center) and the UCSF Center for Excellence in Primary Care (CEPC) are delivering Primary Care Assessment, Coaching, and Technical Assistance (ACT) across the HPSM network. ACT brings together clinical, operational, and population health leaders to strengthen the core capabilities that support better quality, more coordinated care, and stronger HEDIS performance.
Our core strategies include:
- Strengthening PHM capabilities and practice culture across the HPSM network through expert-led training and peer-to-peer learning.
- Conducting comprehensive assessments representing >90% of assigned lives across primary care practices.
- Providing 1:1 practice coaching to 15+ high-volume primary care practices, ensuring strong engagement throughout the intervention period.

What’s happening now
Practices across the HPSM network are completing structured population health capability assessments and meeting with The Learning Center coaches to identify priorities for improvement. Network-wide peer learning and on-demand eLearning give practices ongoing access to the same tools and expert content, while a smaller group of high-volume practices receives intensive one-on-one coaching tailored to their specific operational challenges. The combination is designed to surface where the network is strongest, where common gaps exist, and where targeted investment will have the greatest effect.
The HPSM Primary Care ACT Program has engaged 23 practice organizations with 45 practice sites with the baseline assessments to understand the Population Health Management Capabilities across the HPSM Primary Care Network.
The practices are varied and include Federal Qualified Health Centers, Large Health Systems and Single Independent Practices. The Program is in the process of rolling out its network wide technical assistance and 1:1 practice coaching support. The first kick off session for technical assistance held on 4/30 and 5/7 provided information to the participating practices about the technical assistance offered by the program. The next phase of technical assistance and coaching support starts June, 2026!
What we expect to learn
Primary Care ACT is designed to do more than support individual practices. By assessing capabilities across the HPSM network and pairing those findings with hands-on coaching, the initiative will identify where shared learning is most valuable, where one-on-one support accelerates progress, and where future primary care investment can have the greatest impact for HPSM members. As the initial assessment period concludes, PHLC and CEPC will share emerging themes with HPSM and the broader network.
