It helps practices assess current care-team roles, responsibilities, workflows, and use of staff capabilities; identify opportunities to strengthen or expand team-member contributions; and select realistic priorities for implementation. Practices can use the resource alongside the deliverable template and rubric to clarify expectations, organize their responses, and develop an implementation plan with defined activities, ownership, training needs, timelines, resources, and measures of progress.
