This editable template supports practices in completing the Data Governance Policy and Procedure portion of the EPT Data Governance and HEDIS Reporting milestone. It guides teams in defining data governance roles and decision-making responsibilities and establishing policies for data access, quality, privacy, security, use, sharing, reporting, retention, and integration. Practices can enter their responses directly into the document and adapt the policy to reflect their organizational structure, systems, data sources, workflows, and compliance requirements.
