This template supports practices in completing the EPT Implement Enhanced Outreach and Engagement milestone. It guides teams through documenting how they identify and prioritize patients for outreach, select communication methods, assign care-team responsibilities, track outreach attempts and outcomes, and engage patients in recommended care or services. Practices also use the template to describe implementation activities, workflow or technology changes, staff training, performance measures, and quantitative or qualitative results. The completed submission should reflect the practice’s own patient population, outreach priorities, communication channels, staffing model, systems, and implementation experience.
