This webinar shares interim findings from the Equity and Practice Transformation Program at the midpoint of the three-year initiative. The session highlights how participating primary care practices are strengthening population health management capabilities, using stratified data to identify disparities, improving access and operational workflows, and advancing quality and patient outcomes. Participants can use the recording to understand early program results, implementation lessons, and the role of technical assistance and payment incentives in supporting practice transformation. The findings were developed in partnership with the California Health Care Foundation.
