This template supports practices in completing the EPT Care Team Assessment and Implementation milestone. It guides teams through assessing current care-team composition, roles, responsibilities, workflows, and use of staff capabilities. Practices use the template to identify opportunities to expand or clarify team-member roles, select priority changes, and develop an implementation plan that addresses activities, ownership, training, resources, timelines, and measures of progress. The completed submission should reflect the practice’s own staffing model, patient population, workflows, operational priorities, and implementation experience.
