This completed example shows how Modern Concepts approached the EPT Adopting Clinical Guidelines milestone. It demonstrates how a practice can select an evidence-based guideline, assess current workflows, identify gaps, and plan changes needed to integrate the guideline into routine care. Practices can use the example to understand the expected level of detail for documenting the guideline source, target population, care-team responsibilities, workflow and technology changes, staff training, implementation activities, and measures of progress. This resource is intended as a reference and should be adapted to reflect each practice’s clinical priorities, patient population, systems, and implementation experience.
