This completed example shows how Capitol Family approached the EPT Implement Pre-Visit Planning milestone. It demonstrates how a practice can prepare for upcoming visits, identify care gaps and patient needs, assign responsibilities across the care team, and communicate relevant information before the appointment. Practices can use the example to understand the expected level of detail for documenting workflow steps, staff roles, data sources, planning tools, implementation activities, and measures of progress.
