These slides are from Session 1 of the HealthBegins Health-Related Social Needs Skill-Building Workshop Series. The session introduces foundational approaches for assessing and strengthening HRSN screening, referral, and service-linkage processes. Practices can use the slides to support team learning, evaluate current workflows, identify implementation gaps, and begin planning improvements to how patients’ social needs are identified and addressed.
