This playbook provides practical guidance for improving childhood immunization data exchange and performance on the Childhood Immunization Status Combo 10 measure. It translates findings from the Childhood Immunization Data Exchange Design Studio into workflow diagrams, checklists, and implementation recommendations that practices and care partners can apply. The resource addresses common challenges, including the absence of a single reliable immunization record, fragmented authorization and record-update processes, manual patient outreach, incomplete data, and limited analytic capacity. Practices, health plans, immunization registries, QHIOs, and other partners can use the playbook to clarify responsibilities, strengthen data-sharing workflows, improve record completeness, and more reliably identify and address immunization care gaps.
