We work with 200+ primary care practices collectively providing care to 2 million+ Medi-Cal enrollees through practical tool development, learning, coaching, and statewide peer exchange.
HPSM Primary Care ACT
We are partnering with the Health Plan of San Mateo to provide holistic program design and implementation around their primary care investment strategy.
Through this work, we are strengthening primary care population health management capabilities through holistic program design & management, expert-led training, and peer-to-peer learning.
Intimate Partner Violence
We are partnering with health plans in California to design a strategy for providing trauma informed universal education, screening and response to Intimate Partner Violence.
We are improving health outcomes through holistic program design & implementation, integration into MCP operations and CalAIM services, educating primary care and maternal health providers, and building partnerships with IPV organizations.
Covered California
We partner with Covered California’s Population Health Investment to build peer & collaborative learning opportunities to help practices meet California Data Exchange Framework (DxF) requirements.
