This template supported EPT practices in preparing their initial administrative KPI submission for the November 2024 deliverable cycle. Practices used it to report baseline results for empanelment, patient-side continuity, and third next available appointment as part of the Empanelment Assessment milestone. The template helped teams organize measure calculations, reporting periods, supporting data, and other information needed for submission. Because this version was created for the November 2024 cycle, practices should use the current EPT KPI Submission Template for active reporting requirements.
