Join the PopHealth Learning Center and our Technical Assistance partners for the Fall EPT Learning Community! Our Fall Learning Communities are an opportunity to connect with peers, reflect on your progress throughout EPT, and plan to sustain your work beyond the program.
Join us for the second of two sessions closing out three years of the Equity and Practice Transformation Program. In Part 2, we look forward. With EPT closing in December, we’ll focus on a practical question: how do you keep the improvements you’ve built? We’ll introduce the MOCHA framework — a simple way to check whether a change will last — and then work through it together in small groups with practices like yours. You’ll leave with a worksheet to start your own sustainability plan, a look at the EPT resources that stay available to you after the program ends, and a chance to celebrate what this cohort accomplished together. We’ll also look at where a change goes next, and how value-based payment can fund the roles that hold it.

Practices are required to attend one Part 1 session AND one Part 2 session. Please register for the sessions you plan to attend and decline the alternative options.
