| Fall EPT Learning Community: Part 1 (Session B)
Join the PopHealth Learning Center and our Technical Assistance partners for the Fall EPT Learning Community! Our Fall Learning Communities are an opportunity to connect with peers, reflect on your progress throughout EPT, and plan to sustain your work beyond the program. Join us for the first of two sessions closing out three years of the Equity and Practice Transformation Program. In Part 1, we look back at what this cohort built together. We’ll share cohort-wide results and then move into small-group storyboard rooms where practices share their own stories: what changed, how they did it, and what it took to get there. You’ll choose which stories you want to hear and have time to ask questions directly. Come celebrate what this cohort accomplished and leave with ideas you can use in your own practice.
