This template supports practices in completing the EPT Adopting Clinical Guidelines milestone. It guides teams through selecting an evidence-based clinical guideline, assessing how well current workflows align with the guideline, and developing a plan for implementation. Practices use the template to document the priority condition or population, guideline source, care-team responsibilities, workflow and technology changes, staff training, patient engagement, performance measures, and implementation results. The completed submission should reflect the practice’s own patient population, clinical priorities, systems, workflows, and experience adopting the selected guideline.
