This template assessment supports practices in completing the EPT Data Governance and HEDIS Reporting milestone. It guides teams through reviewing how the practice accesses, uses, manages, shares, reports, and integrates internal and external data needed to calculate quality measures and key performance indicators. Practices can use the assessment to document current data sources, reporting processes, system capabilities, data-sharing relationships, and gaps that affect HEDIS-like measure reporting. The completed assessment should reflect the practice’s actual data infrastructure, workflows, partnerships, and reporting capabilities. This assessment is one of two required deliverables for the milestone and must be submitted with a separate Data Governance Policy and Procedure.
