These slides are from the EPT June Learning Community focused on advancing core components of the EPT care model. The sessions support practices in reflecting on program progress, assessing and strengthening care-team models, adopting and operationalizing evidence-based clinical guidelines, and designing effective pre-visit planning workflows. The resource also includes opportunities for practices to learn from peer implementation experiences and identify practical next steps for strengthening care delivery. Practices can use these slides to support team discussion, implementation planning, and completion of related EPT milestones.
