The Population Health Management Capabilities Assessment Tool, or PhmCAT, helps primary care practices assess their current population health management capabilities and identify priorities for improvement. The 50-question assessment is intended to be completed by a multidisciplinary team and covers eight domains: leadership and culture, the business case for population health management, technology and data infrastructure, empanelment and access, care teams, patient-centered population-based care, social health, and behavioral health. Practices can use their results to support team discussion, track progress over time, and guide practice transformation and implementation planning. EPT practices complete the PhmCAT annually, with at least three staff members participating and consistent team participation encouraged from year to year.
