Welcoming our Chief Program Officer and Chief Operating Officer

The Population Health Learning Center (“Learning Center”) announced today that it is welcoming two key members of its founding executive team. Tammy Fisher, MPH will become Chief Program Officer on March 11th and Eric Hernandez will assume the role of Chief Operating Officer on March 18th.

Tammy Fisher will lead the technical assistance work of the Learning Center. Tammy has extensive experience implementing successful multi-payer practice transformation initiatives in primary care in California and nationally. This includes work in the Medi-Cal delivery system as well as private practice settings and integrated health systems. Throughout her career, Tammy has played leadership roles in care redesign, implementing value-based care, training and facilitation, program development and execution, and healthcare operations. Most recently, she was the Senior Director of Clinical Quality at the Accountable Care Organization Aledade, Inc., where she led national practice transformation and quality strategies. Tammy has also held leadership roles at Contra Costa Health Services, the Center for Care Innovations, Partnership HealthPlan of California, and the San Francisco Health Plan.

Eric Hernandez will lead the core operations of the Learning Center. Eric is a seasoned, results-driven healthcare executive with a track record of efficiently operating healthcare organizations and delivering exceptional patient experience. He has successfully built new healthcare organizations, teams, and programs in California that are focused on supporting low-income communities. Eric is a skilled leader, supporting the growth and development of staff, and building high-performing operational teams that ensure regulatory compliance and deliver quality outcomes. Most recently, he was the Chief Operating Officer at Peach Tree Health, a $55 million federally qualified health center in Marysville, CA that serves over 33,000 patients annually. He is fluent in Spanish.

The Population Health Learning Center is a new organization that aims to achieve impact at scale through population health transformation in Medi-Cal (California’s Medicaid program) and adjacent safety net programs. This includes aligning key funders and initiatives to implement an evidence-based model to build capacity for population health practice, and to advance health equity in California. For the next five years, the Learning Center will serve as the Program Office for the Equity and Practice Transformation Program, a statewide $650 million Medi-Cal program to transform primary care and advance health equity for Medi-Cal enrollees. The Learning Center’s vision is to build a just and equitable healthcare system for all Californians.
