A Collaborative and Evidence-Based Approach to Advancing Population Health Practice
It’s time to scale population health statewide.
California shines in diversity and innovation, but inequities in race, income, and geography persist in our communities. Efforts to transform our health care delivery and reimbursement models toward patient-centered, value-based care are fragmented, leaving primary care practices overwhelmed and confused by various efforts that compete for their attention.
That’s why the Population Health Learning Center (Learning Center) is working to align primary care delivery transformation efforts across CA practices, payers and purchasers to advance population health capabilities, patient outcomes, and address health inequities at scale. As a new organization, we want to be part of building an equitable health care system that works for all Californians. Our approach focuses on safety net systems and low-income communities and is closely aligned with Department of Health Care Services (DHCS) Medi-Cal program priorities and programs.
