High-quality support for effective population health management

The Learning Center is on a mission to scale primary care transformation across California.

Our coaching packages support California's diverse healthcare ecosystem and communities to achieve improved population health equity. Coaching enables practices to close care gaps through outreach and planned care as well as integrating behavioral and social health screening and response workflows.

We offer two types of coaching:

  • Number-1

    General practice coaching

    Provides support to build or strengthen care team capabilities across the population health building blocks, including empanelment, access, data, and models of care. Practices select a population of focus, such as adults with chronic conditions, children and youth, or people living with behavioral conditions health. Practices also select a pilot site to apply the evidence-based approaches through small tests of change.

  • Number-2

    Specialized practice coaching

    Provides targeted and deep technical support from subject matter experts to improve performance on specific quality measures, like childhood immunization status, or selected population health management building blocks, including empanelment, access, data, and models of care. Similar to the general practice coaching, practices receiving specialized coaching select a pilot site to apply their learnings and then scale appropriately.
