Skill Building Workshop – Closing the Loop on Referrals

One of the most frequently asked question – how to close the loop through linkage and documentation. Facilitators will discuss how to establish feedback channels w/ community partners, QHIOs & closed loop linkage platforms (ie FindHelp) to gather the linkage outcome values and methods to seamlessly integrate known closure reasons in the EHR. Additionally, there will be guidance on follow up actions for patients with unknown linkage status.
