EPT – October Learning Session – Redwood

Welcome to the information page for the Equity and Practice Transformation (EPT) program October 2024 Learning Session (Redwood)! Learning Sessions are 4-hour virtual events that happen three times a year to convene EPT practices as Learning Communities.

Please use this registration page if you are assigned to the Redwood Learning Community. EPT practices: login to PopHealth+ or download the Group Assignments spreadsheet to find your EPT Learning Community.

The October 30th Learning Session is divided into two tracks: Empanelment and Access. If your practice has three or fewer attendees, your team will choose to attend either the Empanelment track or the Access track. Practices with four or more attendees may register for both tracks and should have at least two team members per track.

Empanelment Track

Led by the UCSF Center for Excellence in Primary Care, the Empanelment track will help your practice to develop a patient centered empanelment policy and to clean and maintain panel sizes.

Practices will select one of two breakout sessions to attend. The first option focuses on calculating Key Performance Indicators for the EPT Administrative Measures (Empaneled Patients, Patient-Side Continuity, Third Next Available Appointment). The second option will support your team to calculate ideal panel size.

Practices that register for the Empanelment track may also attend an optional, final session presented by data experts from Alliance Chicago. This session will prepare your team to use EHR data and automated reports to calculate the EPT Key Performance Indicators on Empanelment and Access.

Access Track

Led by Coleman Associates, the Access Track focuses on improving capacity utilization and continuity. In this session, practices will be able to experiment with real time schedule management. Practices will also learn how to evaluate the root causes of no-shows, analyze the impact of no-shows, and discuss strategies to improve access to care and maintain patient continuity. The Access track is approved for 0.4 CEUs and 4 hours of continuing education (CE). Click here for more information on CEUs and CEs.
