Case Study: Improving Referrals to Medically Tailored Meals through data sharing integrations

A detailed user story to test improving closed loop referrals to community supports (e.g. Medically Tailored Meals/MTM). Petaluma’s use case outlines referral initiation at the practice, MCP authorization, and service extension—so practices can streamline and automate MTM workflows while improving transparency and turnaround time across partners. The user story details current state and ideal state with proposed solutions to improve streamlined referrals and closed loop linkage. Artifacts were developed in partnership with Connecting for Better Health and made possible through funding from Covered California.
