EPT practices can submit deliverables to meet the following milestones during the May 2025 submission cycle:

• 2025 Population Health Management Capabilities Assessment Tool (PhmCAT)

• Data Implementation Plan

  • Note: Before completing this Milestone, EPT practices must successfully submit the Data Governance and HEDIS Reporting Assessment and Policy and Procedure.

• Stratified HEDIS like-measures

• Access and Empanelment Key Performance Indicators
