How Better Data Exchange can Improve Performance on the Postpartum Care Measure

This webinar shares key learnings and implementation insights from a six-session Postpartum Care Data Exchange Design Studio led by Connecting for Better Health (C4BH) and the Population Health Learning Center (PHLC) and funded by Covered California. The design studio brought together stakeholders across the social and healthcare system, including FQHCs, managed care plans, a labor & delivery hospital, representatives from DHCS, maternal health consultants, and a doula agency to examine real-world barriers to postpartum care coordination.
This webinar is for providers, health plans, and partners interested in implementing data exchange solutions to improve performance on the Postpartum Care (PPC) measure as part of the DHCS Equity & Practice Transformation (EPT) program and advance timely, coordinated, and high quality postpartum care.
Participants will learn:
- Key challenges impacting PPC measure performance, including: confusion around billing and reporting, gaps in data exchange for timely transition to postpartum care, gaps in data exchange for care coordination, and fragmentation between maternal and infant care.
- How clarity around billing and improved data exchange can support timely follow-up and better care coordination
- Practical recommendations and implementation guidance from real-world use cases and workflow redesign
