Led by the Learning Center, UCSF Center for Excellence in Primary Care (CEPC), and our Clinical Population of Focus Subject Matter Experts, this interactive session will support practices in advancing core components of the EPT care model and connecting real-world experience to clinical transformation.

This session will focus on:
Key strategies for implementing clinical guidelines, building effective registries, and aligning care team roles with population needs.
How to strengthen planned care and outreach efforts through workflow design, technology, and team-based approaches.
Connecting Journey Maps to care model elements to deepen understanding of patient experiences and disparities.
Practical examples of how registry and care team infrastructure support care gap closure and equity.
