June Learning Community Part 1

Led by the Learning Center, UCSF Center for Excellence in Primary Care (CEPC), and our Clinical Population of Focus Subject Matter Experts, this interactive session will support practices in advancing core components of the EPT care model and connecting real-world experience to clinical transformation.

The June Learning Community will support practices in advancing core components of the EPT care model and connecting real-world experience to clinical transformation, including:
-Reflecting on program progress and planning for upcoming work
-Assessing and strengthening care team models
-Adopting and operationalizing clinical guidelines
-Designing and implementing pre-visit planning workflows
-Collaborating and sharing implementation strategies with peer practices
