PhmCAT Support Session

This session will offer hands on support in understanding and completing the annual Population Health Management Capabilities Assessment Tool, known as the PhmCAT. A facilitator will guide participants through the PhmCAT sections, sharing best practices, checking for understanding and answering questions. This is an optional session, which will be offered multiple times between March and April 2025.

To maximize the learning experience at the Learning lab, participants should review the PhmCAT in advance. Please come prepared to ask questions.
