Skill Building Workshop – Intervention & Linkage to Services

This session dives into standardized intervention and linkage protocols and best practices. Facilitators will provide tools for partnership engagement for high priority needs, such as food, housing, transportation, and safety resources. This session will also highlight opportunities to leverage CalAIM services lines (ECM, CS, CHW) to help fill care gaps through warm hand-offs to comprehensive care coordination services.
