Making Practical Progress in Population Health and Equity 

In this report — co-sponsored by the California Health Care Foundation (CHCF) — the Learning Center shares data at the midway point of the three-year Equity Practice Transformation (EPT) program 

Highlights include: 

  • Putting data to work for equity in care — Using stratified data by race, ethnicity, and other factors to identify disparities and target solutions. 
  • Improving access and efficiency — Streamlining workflows and reducing barriers so patients can more easily get the care they need. 
  • Driving better outcomes — Demonstrating measurable improvements in health and experience for patients, while also renewing satisfaction for providers.

The Equity Practice Transformation program is a large-scale partnership between the PopHealth Learning Center and California’s Department of Health Care Services (DHCS) to build population health management capacity in Medi-Cal primary care, supported by a $140 million investment by DHCS.
