Postpartum Care Community Design Studio: Final Artifacts and Implementation Planning
Connecting for Better Health, in partnership with the PopHealth Learning Center, led a co-design sprint with EPT practices, MCPs, and DHCS to draft common user stories and workflow solutions to improve postpartum care. The user stories highlight current state barriers and future state workflow and data sharing opportunities to enhance quality PPC measure performance and close care gaps.
User Story – Yolanda Ortega
This user story provides a narrative of Yolanda’s postpartum care journey, from the delivery of
her baby via C-section to her postpartum visit in the hospital to her hospital admission for
gestational diabetes to her 4-week postpartum visit.
User Story – Aliyah Jackson
This user story provides a narrative walkthrough of Aliyah’s postpartum care journey. It includes
her delivery in the hospital, her hospital admission for preeclampsia, and her 1-week
postpartum care visit.
