Meet our cadre of experts helping to advance population health management capabilities and outcomes across California as part of the Equity and Practice Transformation (EPT) Payment Program.
Leveraging complementary expertise at scale
We’ve brought together best in class state and national experts to design the EPT primary care program curriculum and deliver technical assistance to participating practices statewide. We are engaging with additional technical assistance partners regularly and will update this page as we bring new partners on board.

How our partners provide support
Our technical assistance partners provide the following types of support as part of the EPT’s technical assistance structure. Our multi-pronged approach is meant to help the diverse cohort of primary care practices strengthen their population health management infrastructure, enabling them to achieve EPT milestones and receive directed payments to invest in care delivery.
Subject matter experts (SMEs)
Develop and deliver content in EPT’s population health management building blocks, including access, empanelment, data to enable population health management, care delivery models (including disparity reduction, social needs, behavioral health), and value-based payment. SMEs provide training on the EPT Key Performance Indicators (KPIs). SMEs leverage modalities that include eLearning, virtual learning sessions, and office hours to support practices in addressing their challenges and in adapting the EPT curriculum to their practice environment.

Practice Track Facilitators
Support EPT practices to operationalize changes from the EPT curriculum through one-hour sessions every two months. Sessions are focused on peer sharing, problem solving around common challenges, and identifying where additional technical assistance is needed.

Coaches
Offer one on one, shoulder to shoulder support to help EPT practices translate the curriculum into evidence-based protocols and workflows that align with their practice culture, improve patient outcomes, and reduce disparities. Coaches help practices test, implement, and measure the impact of changes on meeting program milestones and deliverables. Coaching for EPT practices is an optional, add-on service.

The building blocks of population health management. EPT practices also use PopHealth+ to complete pre-work for program events, download templates and job aids, and participate in group forums.
