EPT Milestones & Deliverables


Program Overview

The Equity and Practice Transformation (EPT) Directed Payment Program is organized around 25 population health management milestones, each one developed in partnership with the California Department of Health Care Services (DHCS). EPT practices are eligible to receive payment(s) for meeting one or more of the 25 EPT Milestones. Evidence of milestone completion is in the form of deliverables accepted by the Learning Center during the May 1 and November 1 submission cycles each program year.

Below you can find templates, rubrics, and other resources to help you prepare to submit EPT Milestones and Deliverables. Please submit all deliverables through the EPT Deliverable Portal.

Unless stated otherwise, the dates below represent the first opportunity for EPT Practice to submit a given deliverable. Please send any EPT program related questions through our contact form. Our team is happy to assist you.

May 2026 Deliverables

EPT Practices can submit both Nov 2025 and May 2026 deliverables during the May 2026 submission cycle. The numbers next to the title of the template indicate the associated milestone number.

What is required to submit in May 2026:

  • 2026 PhmCAT,
  • Data Policy & Procedure,
  • Empanelment Policy & Procedure,
  • Data Implementation Plan,
  • Disparity Reduction Plan, and
  • At least one Model of Care deliverable (e.g. pre-visit planning, outreach and engagement, care team assessment, or adoption of clinical guidelines)

To meet this milestone, practices must have at least three staff members complete the PhmCAT survey online. When possible, please have the same staff members fill out this year’s PhmCAT as did last year’s PhmCAT, since the results will be trended.

Beginning 2025, EPT practices will fill out the PhmCAT in the EPT Deliverable Portal. Please contact us(opens in new tab) if your staff members need access to the Deliverable Portal.

Updated May 2026

To meet this milestone, practices must have at least three staff members complete the PhmCAT survey online. When possible, please have the same staff members fill out this year’s PhmCAT as did last year’s PhmCAT, since the results will be trended.

To meet this milestone, practices will implement behavioral health screening and linkage and implement health-related social needs (HRSN) screening and linkage.

Every submission cycle starting Nov 2024, EPT Practices submit KPI reporting for the Empanelment, Continuity, and Third Next Available Appointment administrative measures as part of Milestones 18, 19, and 20. Please note that milestone completion through either achievement or improvement benchmarks must be sustained over two consecutive submissions or met in the final submission for payment eligibility.

The following reporting template helps EPT Practices prepare to provide the KPI reporting in the Deliverable Portal as part of the bi-annual submission process. In addition to the Empanelment, Continuity, and Third Next Available Appointment administrative measures, the KPI Assessment also asks practices to provide their current Medi-Cal Assigned Lives as well as performance on quality of care metrics specific to their Population of Focus (PoF).


November 2025 Deliverables

EPT Practices can submit both Nov 2025 and May 2025 deliverables during the Nov 2025 submission cycle. The numbers next to the title of the template indicate the associated milestone number.

What was required to submit in November 2025:

  • 2025 PhmCAT
  • At least one 2024 deliverable that has not been submitted or accepted (e.g., empanelment assessment, empanelment policy and procedure, data assessment and policy and procedure)

What is required to submit in May 2026:

  • 2026 PhmCAT,
  • Data Policy & Procedure,
  • Empanelment Policy & Procedure,
  • Data Implementation Plan,
  • Disparity Reduction Plan, and
  • At least one Model of Care deliverable (e.g. pre-visit planning, outreach and engagement, care team assessment, or adoption of clinical guidelines)

To meet this milestone, practices will adopt clinical guidelines, implement enhanced outreach and engagement, and implement pre-visit planning.

To meet this milestone, practices must submit a report on their performance across all the HEDIS®-like quality measures for their selected Population of Focus (PoF). The reporting will be stratified by race and ethnicity and at least one additional characteristic: primary spoken language, sexual orientation, gender identity, housing status, or disability status.

For this milestone, practices must develop an implementation plan to address data and technology gaps and transform practice operations to support development of KPIs.

November 2024 Deliverables

To meet this milestones, practices must assess their current empanelment environment including understanding of baseline data on percent of patients who are empaneled to a provider/care team, continuity based on current assignment, and third next available appointment.

To meet this milestone, practices must develop and implement a standard policy and procedure that addresses the method of assigning patients to care team panels, changing assignments, maintaining panel size and continuity, and monitoring empanelment effectiveness.

To meet this milestone, practices must develop a data governance policy and procedure and assess how the practice is accessing, using, managing, sharing, reporting, and integrating data from external sources that are required to produce KPIs for the selected population. Please note that this milestone requires the submission of two distinct deliverables, the reporting assessment and the data governance policy and procedure.
